The definitions of menstrual irregularity and bleeding irregularity were changed by FIGO (World Society of Obstetrics and Gynecology) in 2011. In the past, terms such as dysfunction uterine bleeding, polymenorrhea, menorrhagia, menometrorrhagia, oligomenorrhea, metrorrhagia were used for menstrual irregularities and bleeding irregularities. The currently used definitions were causing a lot of confusion. All menstrual irregularities and bleeding irregularities are now called Abnormal Uterine Bleeding.
Abnormal Uterine Bleeding It can happen in 2 ways.
Abnormal uterine bleeding is seen in 10% to 30% of women of reproductive age. To understand what abnormal uterine bleeding is, it is first necessary to know the normal menstrual pattern.
Normal Menstrual Cycle Characteristics. (Features of Normal Menstrual Pattern)
- Menstruation occurs between 21-35 days.
- Menstrual bleeding lasts between 4-8 days.
- The amount of bleeding occurs between 4-80 ml.
The periods when ovulation time, that is, egg cracking, does not occur in a healthy way, that is, cannot be predicted, are pre-menopausal and adolescence periods.
Abnormal uterine bleeding is also divided into 2 according to the time it occurs.
Acute abnormal uterine bleeding is bleeding that requires rapid intervention and the amount of bleeding is intense.
Chronic abnormal uterine bleeding is bleeding that lasts longer than 6 months.

How is Abnormal Uterine Bleeding Diagnosed?
First of all, the patient’s medical history should be listened to carefully. The amount and duration of bleeding and whether bleeding occurs at the end of menstruation, during menstruation, or between two menstrual periods should be questioned. Bleeding after intercourse should be questioned. If the complaint of abnormal uterine bleeding begins in the adolescence period, when menstruation begins, the patient should be questioned about bleeding disorders and the medications used, if any, should be noted.
Some blood tests must be done for diagnosis
- Pregnancy should be excluded. b-hcg should be checked in the blood.
- A hemogram test should be performed to determine the severity of anemia caused by uterine bleeding.
- To evaluate hormonal disorders that may cause bleeding; FSH, LH, TSH, Prolactin hormones should be studied.
The uterus should be evaluated.
- The uterus is evaluated with ultrasonography. The uterus should be carefully evaluated for polyps, myomas, hyperplasia, adenomyosis and possible cancer. If necessary, a biopsy should be taken from the inner lining of the uterus, that is, the endometrium.
- An ultrasound procedure, which we call saline infusion sonohysterography, can be performed by injecting serum into the uterus. In this procedure, lesions that occupy space in the uterus, such as polyps and myomas, are better evaluated.
- Hysteroscopy; It is the gold standard for evaluating intrauterine lesions. It is the most useful diagnostic tool. It can also be used for treatment.
Treatment for Abnormal Uterine Bleeding
Surgery should be performed in the presence of space-occupying lesions such as myomas and polyps. Depending on the size, severity, age and cancer risk of the patient’s lesions, removal of the existing lesion or hysterectomy may be necessary.
In cases of bleeding due to hormonal disorders, medical treatment is often beneficial. In such patients, surgery may be considered when medical treatment does not work.
